Care Coordination
We assign a care coordinator to a client for the duration of any Toronto Community Care Access
Centre service. The person's needs are assessed by the assigned care coordinator together with the person
themselves, with the goal of achieving or maintaining the client's optimal capacity to manage at home
or at school. As needs change, the plan of service is adjusted. The coordinator arranges for the needed
service to be delivered by a member of staff or a contracted service provider working on behalf of the Toronto Community Care
Access Centre. Or, the coordinator assists with the application and admission of the person to other
community services or to a long-term care facility.
Information
& Referral
The Toronto Community Care Access Centre will provide you with information
on services provided by community agencies, as well as referrals to these services.
Services may include, but are not limited to legal help, dental clinics, housing
resources, Meals on Wheels, tax services, drop-in centres, day programs, culturally-specific
services, and extreme cleaning services. The following is information on specific

Placement
The Toronto Community Care Access Centre's Placement Services process is
fair, providing equality of treatment to all clients. It is a process that is
culturally sensitive and respectful of the dignity and choices of each individual.
All nursing homes and homes for the aged across the province offer standardized levels of care. Residents in a long-term care facility are expected to pay the monthly fee determined by the Ontario Ministry of Health. Those who cannot pay the full fee, but qualify for financial assistance, can still be considered for admission.

How to arrange placement
When
you or the person you care for is ready for placement, or even if you just have
questions, call us. We will put you in contact with a staff member who will
assist you with the various steps necessary. Our staff will work with you and
your support network to provide all the information you require.
Your choices and preferences guide the process. We work with you to plan the placement that is best for you or the person you care for. If your first choice is not available our staff will help you find an appropriate alternative.
If you need services while waiting for placement, we will make the necessary referrals and connect you to the appropriate services and service providers.

Eligibility
To determine if you are eligible to receive our services,
we can arrange an assessment for you. We'll help you coordinate
a plan that's responsive to you, our client, and your caregiver or
family support system.
What You Need to Receive Our Services
Who Pays for Services
All services provided to eligible clients by the Toronto Community Care
Access Centre are paid for by the Ministry of Health and Long-Term Care.
We will also help you to find other community-based services you may need,
or put you in contact with agencies or suppliers you pay for yourself.
Working Together
The Toronto Community Care Access Centre works in partnership
with you and your support team to assess your needs and coordinate your care.
Together we will explore the community resources that are available to help
meet your needs.
