Making a referral

Referrals for Community Care Access Centre programs and services may be made by physicians, other health care professionals, patients, or their family, friends, or neighbours. Referrals may be made by call the main telephone number for Client Services at 416-506-9888.

For referrals which require medical orders including dressings, injections, insulin and IVs, professional tasks (e.g., nurse to inform MD on critical blood pressure readings and glucose levels), a medical referral form (see link below) is needed. This form needs to be completed and returned by fax using the Referral fax cover sheet (see link below). Your office must call the main telephone line 416-506-9888 to inform us that a fax is being sent, especially if you require same day or urgent (within 24 hours) services provided.

For referrals that do not require medical orders (e.g., Occupational therapy assessment for home safety) you may make telephone referrals at 416-506-9888.

Eligibility Criteria

A valid Ontario Health Number.

A need that cannot be met by hospital outpatient services or other community programs.

A need that can be met at home with services from the Toronto Community Care Access Centre.

A living situation that allows service to be provided. Support from family or friends to assist with treatment if necessary.

Billing OHIP

Please note that the Ontario Ministry of Health and Long-Term Care has provided billing codes for the completion of home care application request forms. Please refer to the Schedule of Benefits manual from the Ministry for these codes. Note also that there are codes relate to Acute and Chronic Home Care Supervision of patients receiving Community Care Access Centre services.
