Acquired Brain Injury
The ABI Program is a community-based rehabilitation
service for people living with the effects of acquired brain injury.
Child and Family Services
The Child and Family Services Program
is a specialized program that provides service coordination and support to children,
youth and their families to assist with short or long-term disability or other
special needs.
Homeless Outreach
A high proportion of this City's homeless and underhoused population is located
in our area. Toronto Community Care Access Centre, working closely with the
Regent Park Community Health Centre, provides homeless
individuals with a range of services. We also work with many Out-of-the-Cold
Programs and participate in the Lice and Scabies Control Program.
Palliative Care
Palliative care is aimed at the relief of suffering and improvement in the
quality of life for persons who are living with or dying from an advanced illness
or who are bereaved.
The Hospice Palliative Care Network Project (HPCNet) offers access to a network of support for persons with an advanced illness who wish to remain at home, or who live in a long-term care facility.
School Health Support
The Toronto Community Care Access Centre provides services
at school for children and youth with special needs such as speech therapy,
tube feeding, medication, or help with printing or writing.
The School Health Support Program also facilitates the provision of health
support services to private schools.
