Long-term Placement Services
IF YOU, OR THE PERSON YOU CARE FOR, reach the point where you need more support than can be provided at home, the Toronto Community Care Access centre will work with you to arrange placement in a long-term care facility that best meets your needs. Your preferences will guide this process and our staff will be there with you to provide information and support.

Short-term Placements
In addition to placement services in long-term care facilities, the Toronto Community Care Access Centre is also prepared to coordinate short-term institutional care. This support might be required when you are recovering from an illness or injury, or it might be needed to provide respite care, allowing some time off for a caregiver.
