  • Help you assess your health care needs.
  • Plan, coordinate and ensure delivery of a full range of in-home services to support clients and their caregivers.
  • Help clients with their applications and admission to long-term care facilities in Ontario.
  • Provide you with information about and referral to a wide range of community health and support services to meet your needs.
  • Provide services in publicly funded Toronto schools for children with disabilities.
  • Provide special services for people with conditions such as Acquired Brain Injury (ABI).
