Palliative care is aimed at the relief of suffering and improvement in the
quality of life for persons who are living with or dying from an advanced illness
or who are bereaved.
A pilot project is underway to develop a new model of home palliative care coordination and service delivery.
The Hospice Palliative Care Network Project (HPCNet) offers access to a network of support for persons with an advanced illness who wish to remain at home, or who live in a long-term care facility.

The HPCNet Project also offers educational opportunities for primary care providers and volunteers to enhance their knowledge and skills in palliative care.
